How to refer

We accept referrals from local authority commissioning teams, social workers, care managers, NHS discharge teams, ICB commissioners, and families making private arrangements.

Send referral information

Send the current care plan, risk assessments, placement history, medical history, and funding confirmation. We will tell you exactly what we need.

We assess and respond

We review the information and give you a clear yes or no within 48 hours of receiving a complete referral. If we cannot proceed we explain why.

Placement confirmed

We coordinate the move in, introduce the support team, and provide you with a named contact for the duration of the placement.
