Process
The Referral Process
The usual process for placement with PHP Care Services follows these steps:
STEP 1 - Initial Contact (Immediate)
Contact us by phone or email with basic referral information. We'll provide initial guidance and send you our referral form if appropriate.
STEP 2 - Formal Referral Submission (Within hours)
Submit the completed referral form with required documentation. We'll acknowledge receipt and provide timeline for review.
STEP 3 - Assessment and Response (24-48 hours)
Our team reviews all information and provides a clear decision with detailed reasoning. If we proceed, we'll outline next steps and timelines.
Additional Services
Required Information for All Referrals
Current Assessment and Care Plan
Most recent social care assessment including needs analysis, risk assessment, and current care plan with goals and objectives. This helps us understand the individual's current situation and support requirements.
Risk Assessments and Safety Plans
Comprehensive risk assessments covering all identified risks, including physical health risks, mental health considerations, behavioural challenges, environmental risks, and safeguarding concerns.
Health and Social Care History
Medical history including current conditions, medications, and healthcare needs. Social care history including previous placements, what worked well, and any challenges experienced. Mental health history and current support arrangements.
Ready to Make a Referral?
How quickly do you respond to referrals?
We provide initial response within hours and full assessment within – hours for standard referrals. Urgent referrals receive prioritized response within – hours
What if you decline our referral?
We provide detailed explanation of our decision and suggest alternative providers where
possible. We’re happy to discuss the referral further and consider future referrals if circumstances change.
Can we visit before making a referral?
Yes, we encourage professionals to visit our services to understand our approach and
environments. Contact us to arrange a visit at a convenient time.
Do you charge for referral assessments?
No, there is no charge for referral assessments. We only charge for actual care provision once a placement begins
What if the individual's needs change after placement?
We conduct regular care plan reviews and adapt our support as needs change. Our flexible
approach means we can usually accommodate changing needs within existing placements.
